Guardian Angel

Did you receive outstanding care from one of our medical team or support staff?  If you would like to recognize them with a Guardian Angel Award simply complete the information below and we will make sure they receive a Guardian Angel Pin and Certificate given in your name.  Contributions are optional but to submit an electronic recognition a $1 donation is required.  If you would like to submit a nomination without fee, contact us at (559)624-2359 or [email protected].  Proceeds are used to provide grant funding for our staff for patient care items.
