As a result of our Community Health Needs Assessment and strategic decision making processes, we are specially focused on: Improved and Continued Access to Preventive and Primary Care for underserved and high-risk populations; Reducing Food Insecurity & Promoting Healthy Nutrition; Expanding Behavioral and Mental Health Access; Enhancing Senior Health, Independence, and Caregiver Support; Strengthening Chronic Disease Management; and addressing other healthcare gaps for vulnerable individuals in our community such as pregnant women, children, veterans and the unhoused.
