We invite you to join our heartfelt mission of mobile health outreach for the homeless, made possible through our partnership with Firehouse Ministries. Every week, a compassionate physician and nurse from our team step beyond the confines of our healthcare center and reach out to those who have no place to call home.

These individuals, recently discharged from the hospital, often struggle to navigate post-care in their challenging circumstances. It's our aim to ensure they don't have to walk through the revolving doors of emergency rooms or face readmission to hospitals due to lack of adequate follow-up care.

But our care doesn't stop there. Beyond the immediate medical aid we provide, we make it a priority to connect them with our larger clinical team. This paves the way for them to receive ongoing medical and behavioral health services. These connections can prove transformative, acting as bridges to a healthier, more secure future.
