JaxCareConnect

JaxCareConnect connects uninsured Duval County residents with safety-net Primary Care - through our network of free and charitable clinic partners.

In 2019, we were born as a project of the Duval Safety Net Collaborative (Agape Family Health, Community Health Outreach, Mission House, Muslim American Social Services, Sulzbacher, Volunteers In Medicine Jacksonville, and We Care Jacksonville, Inc.). Today, as a proven program, we are hosted and administered by WeCareJax for the community. 

Since our inception, JaxCareConnect has served over 6,000 uninsured neighbors, helping them get established with a Primary Care clinic and additional resources that address other barriers to care.
