The Carmella Rose Health Foundation serves the underserved and often invisible populations of the city of Cleveland. We meet our clients where they live, eat and pray at homeless shelters, pantries, and community drop-in centers. We identify health-related and social service needs and provide intensive community navigation to those at high risk...... one client at a time!

Here's an example of just one of the lives we've impacted, and your generosity will help us continue our mission!

We met Andre, an extremely outgoing person at the Men’s Shelter. He was recently released from prison after serving several years. When we met Andre, he was uninsured.

We assisted him in enrolling in Medicaid and Supplemental Nutrition Assistance Program (SNAP). We educated him on the managed care organizations available to him and helped him enroll. We reviewed the benefits of his choice for a Managed Care Plan…one of which was qualifying for a laptop so that he could look for employment! Now that Andre had Medicaid, he was eligible for a free government phone…which we ordered for him. Andre also needed assistance in finding a primary care provider. We were happy to set up an appointment for him to establish care and regain his health!

We are thrilled and humbled to have played a role in Andre’s journey. He says he still has a way to go but he is so grateful for the things that he has accomplished so far.
