We will serve as community health workers that will address the needs of the community. We are supporting the community by meeting individuals where they are to improve the gap between health deficits health knowledge. We offer bilingual support, as well. We hope to reach the goals as listed below. We will meet with members on site and in the home. We aim to treat the person as a whole to improve outcomes and reduce poor outcomes by providing support and education that is needed.
Our vision is to connect with healthcare providers for direct referrals. We will also connect with faith based organizations, community centers and community engagement to inform about our free service. As licensed professionals, we will utilize our skills to assess, educate and care for our community. We want to provide support as professionals, but allow our clients feel and know that they are being heard and they do not have to navigate health and wellness alone. During these assessments we will often find other concerns that may interfere with their well-being. So, we will be there to support and assist with finding resources. For example, our healthcare service will include wellness checks, monitoring vitals, diabetic foot checks, nutritional education and disease management & prevention. We will help coordinate care with primary care and specialists.
Goals & Objectives:
1. Improve Healthcare Outcomes:
Objective: Assess primary care and mental health needs, and connect with services to 125 community members reducing health disparities.
2. Advance Nutritional Education:
Objective: Conduct bi-weekly nutritional educational workshops and include 12 cooking demonstrations, reaching at least 100 individuals for the year.
