Welcome to The CP Family Transition House, where we support children and youth on their journey to self-sufficiency.

The CP Family Transition House is dedicated to helping youth in care navigate the challenging transition to adulthood. For many young people who have experienced foster care, this transition can be especially daunting, as they often lack the safety net and support systems available to their peers. Our program is designed to bridge this gap, providing the guidance and resources needed to help youth in care successfully move into adulthood.
