To minimize the risk of COVID-19 infection, you must follow established protocols, including posted instructions and the Centers for Disease Control and Prevention (CDC) Guidance, while visiting Alpha Kappa Alpha Sorority, Inc.®, Omicron Eta Omega Chapter events and activities.

I acknowledge that participation in Alpha Kappa Alpha Sorority, Inc.®, Omicron Eta Omega Chapter events and activities that bring people together carries the risk of COVID-19 infection, both to myself and others present. I do hereby assume responsibility for my own well-being. I will follow posted instructions and CDC Guidance while visiting Alpha Kappa Alpha Sorority, Inc.®, Omicron Eta Omega Chapter events and activities. I hereby consent in advance to leave and exit the event, without protest or refund, upon request by Alpha Kappa Alpha Sorority, Inc.®, Omicron Eta Omega Chapter, due to my refusal to follow established protocols required by Alpha Kappa Alpha Sorority, Inc.®, Omicron Eta Omega Chapter, the venue provider, the event sponsor, and governing authorities.

I agree that my refusal to follow established protocols provides sufficient grounds to be excluded from attending the event by Alpha Kappa Alpha Sorority, Inc.®, Omicron Eta Omega Chapter or an event authority.
