For each tribute, the teacher or staff member of your choice will receive your personalized note along with a chocolate star. These will be delivered in June to the teachers. 

Each tribute has a suggested tax deductible donation of $15 which goes to funding educational grants throughout the district of the Chatham's. 

Note: if you have more than 6 tributes, please submit the form twice.
