Join us in the fight against diabetes in our vibrant community of Taos! Taos Whole Community Health is launching a groundbreaking initiative – a free, year-long CDC certified Diabetes Prevention Program. This program is designed to address the rising prevalence of prediabetes and diabetes in our community, providing a proven and reproducible solution to empower individuals to take control of their health.
The Challenge:
In New Mexico, approximately 34% of the population is prediabetic, and 12.3% have diagnosed diabetes, with an additional 3-4% undiagnosed. These numbers underscore the urgency of our mission to prevent the further spread of this debilitating disease.
Our Solution:
Taos Whole Community Health aims to implement a comprehensive program modeled after the CDC's successful initiatives in Santa Fe and Albuquerque. We've laid out a detailed plan to achieve CDC certification and train lifestyle coaches. This program, once proven successful, will be eligible for sustainable funding from the NM Department of Health, private insurance, Medicare and Medicaid but we need your help to prove how beneficial this program would be for the public.
Our success is measured by creating a CDC certifiable program addressing prediabetes in Taos. We aim for a 90% success rate for individuals completing the program, defined as losing at least 5% of overall body weight or decreasing A1C out of prediabetic range.
