Every day in North Jersey, people are living in their cars after a full shift at work. Seniors are skipping medication because they can’t afford it. And too many neighbors are being left out of healthcare programs that already exist — simply because they don’t know how to access them.

HS&H Outreach Services is showing up — and standing up — for the individuals that revitalization often forgets.

Over the past 14 months, we've sustainably housed more than 100 individuals through our wrap-around supportive housing program. But we don't stop at sustainably housing our neighbors. We take on the hardest cases across Passaic and Essex Counties, offering real housing solutions, clothing, food, prescription support, and direct connections to healthcare and social services.

We are also one of the few public nonprofits in New Jersey certified to assist individuals with Social Security paperwork. To date, we've helped nearly 200 clients gain access to the benefits they’ve earned but have struggled to receive.

Serve the over-employed and under-housed, including individuals sleeping in their cars despite holding jobs
Connect seniors and adults 50+ with Medicare, Medicaid, and underutilized benefits
Assist clients with Social Security applications and paperwork — helping more people secure long-overdue benefits
Deliver dignity-focused services — clothing, food, prescriptions, and hygiene items
Expand sustainable housing placements with follow-up care and life resources
Bring in-person outreach and resource navigation directly to areas of highest need
