We met for what could be his last birthday. We sang. He blew out the candles. One thing I quietly noticed afterward: no one seemed eager to eat the cake.

He hadn't been tested. Not because he didn't know the risk. Because walking through that door -- being seen going there -- felt more dangerous than whatever he might find out.

1 in 3 people who test positive for HIV already have AIDS when they find out. That number exists because millions of people make the same calculation he did. And the system never notices they're gone.

Unity Health Foundation is building infrastructure that removes that calculation entirely. Anonymous entry. No fixed address. Care deployed where transmission is highest, not where a clinic lease exists. A system structurally incapable of betraying the people it serves.

One test changes everything. A negative result opens the door to prevention. A positive result caught early means AIDS is never part of the story. The only result that leads nowhere is the one that never happens.
