Jessica and Eleanor have taken on a fundraising challenge for DocWayne and will be hosting a tennis tournament and dance bash to support mental health awareness.

DocWayne is a nonprofit organization providing brighter futures to youth through sport-based therapy to improve mental health. Doc Wayne confronts the stigma on mental health by providing the training and services to address barriers of race, gender, and trauma in order to provide equitable access to mental health for all.

With PositiveTracks we work towards DocWayne’s mission by encouraging movement and collecting funds to support mental health. Support Jessica and Eleanor’s event by achieving PositiveTracks’ invite to “Sweat for Good!”

Our goal is to raise $2000 to support DocWayne and the 500,000+ youth impacted.

In consideration of my participation in the Tennis Tournament and Dance Bash and all related activities, I hereby assume all risk of injury, damage, loss and/or expense on behalf of myself, my heirs, executors, administrators, and personal representatives. I release and hold harmless the organizers, sponsors and volunteers involved in the planning and operation of the Tennis Tournament and Dance Bash from liability for any injury to me or my property and any and all claims in any manner arising from or associated with my participation, whether the liability, loss or damage is caused in whole or in part by their failure to use reasonable care in their activities associated with the Tennis Tournament and Dance Bash.

If I am under the age of 18 years, this release and hold harmless agreement shall be signed on my behalf by my parent or guardian.

In addition, I hereby give full permission for the photo/video/film use of my name and image in connection with this event.
