Elitone Donation (in-kind)

Donate your used Elitone device and unused GelPads. You will help us provide vital pelvic health treatment to women in need. Receipts will show $160 in value for Elitone devices and $20 for each GelPads pack < 1 year old for tax purposes.📝

1. Choose "$1 In-kind device donation"
2. Select “Mail a check” at checkout (you don't mail it, but it allows pass-through checkout). 
3. Package and ship your donation to the address at the end of this form. 
4. You will receive a receipt for tax purposes (no value), and an updated value receipt after the donation is received.
