The Challenge

Every day in Lansing, older adults and people living with chronic illnesses are facing housing crises — sleeping in cars, bouncing between motels, or being discharged from hospitals with nowhere safe to go.

Many are managing serious health conditions-heart failure, diabetes, kidney disease- while also trying to survive without stable housing or consistent medical care.
For too many, the gap between “discharge” and “safe home” becomes a revolving door between hospitals, shelters, and the streets.

That’s where DAP Services & Resources steps in.

Our Mission

Founded by a nurse who refused to look away, DAP exists to make sure no senior is left behind.
We provide care coordination, housing support, and resource facilitation for older adults and individuals managing chronic health issues — especially those experiencing homelessness or housing instability.

Our programs connect clients to shelter, benefits, transportation, medication management, and ongoing healthcare support.
Through our Senior Stability Program, we help them move from crisis to independence with dignity, compassion, and consistency.

Why Giving Tuesday Matters

This Giving Tuesday, we’re raising $10,000 to help 30 seniors in our community secure the stability they deserve this winter.

Provides basic personal care and hygiene essentials for one senior entering housing or shelter.
(Think: toiletries, socks, cleaning items, comfort supplies.)

💛 $50 -Fuels Connection

Helps a senior access critical appointments and community resources through transportation support or phone/data access for case coordination.
(Because staying connected means staying safe and supported.)

💛 $100 -Stabilizes a Transition

Contributes to a Senior Stability Starter Kit-covering part of the cost of move-in supplies, groceries, or medication assistance as clients enter new housing.
(It’s a bridge between crisis and security.)

💛 $250 — Provides One Month of Wraparound Support
Contrubites to a full month of care coordination, transportation, and case management for a senior client in transition.

💛 $500 — Sponsors a Senior’s Journey to Stability
Contributes to one month of housing and essential services for a senior or adult with chronic illness.

💛 $1,400 — Sustains Stability for a Family in Crisis
Funds the housing, care coordination, and ongoing support needed to help a senior or disabled adult maintain stability while awaiting long-term placement.
