No income restrictions. You do not have to reside in Lewis County. Cats/kittens must be over 5 months old

Cats MUST BE FRIENDLY for this clinic

The clinic requires: Cat's name, approximate age, long hair/short hair, color of cat, if friendly or not (do not skip this step)

EACH cat MUST arrive in their own HARD SIDED PLASTIC cat carrier with a towel inside that you do not care about (you might not get the same one back) - Carrier needs to be large enough for your cat stand up, lay down comfortably - If you do not have a proper cat-sized carrier, please reach out to pick one up. We have many available to borrow.

No collars or leashes. No toys or sentimental items (you will not get them back). No food or water in the carrier. Just a cat and a clean towel inside the carrier

There will be 24 cats on this transport. Please be respectful of space on the bus. Carriers should be cat-sized

Liability Waiver

I acknowledge that transport and veterinary procedures carry inherent risks, including injury, escape, loss, or death. I assume all risks and agree to hold harmless Onalaska Farm Sanctuary (OFS), its volunteers and agents, from any liability arising from transport or care, including injury, loss, or death of the animal.

I further agree that OFS is not responsible for any additional or emergency veterinary care or related costs.
