CONCRETE ROSES MENTORSHIP

Concrete Roses is an 18-month cohort designed to educate and empower survivors who want to overcome challenging times. This intensive cohort is especially designed for women and men 18 and up who have been impacted by violence, human trafficking, abuse in foster care, substance use, mental unwellness, and recidivism. With the guidance of community health workers, peer supporters, and advocates, participants build the skills and confidence to enhance their quality of life. Just $1,500 can sponsor a participant through this life-changing cohort. 
Together, we will break cycles and cultivate resilience.

To accomplish this goal each participant creates an individualized care plan in addition to health education, care coordination, mentorship, social engagement, and workforce development. Using trauma informed evidence-based practices we work with participants to identify and address social determinants of health walking beside them as they reach milestones and strengthen their connection to self, family, and community . A second chance at life is the best way to describe our cohort. Your support saves lives and restores families.
