Please check your email or online at https://www.pvastx.org/spayandneuteror your waiver PDF and clinic-day instructions, which include your check-in time, drop-off details, and required items. Be sure to:

Print and sign your waiver before your appointment for fastest check-in.

Provide proof of Rabies vaccines, or be prepared to purchase them at check-in.

Have your pet secured with a leash or in a carrier—no loose pets will be accepted.

Bring your payment receipt (digital or printed).

Withhold food after midnight the night before surgery (water is fine until the morning).

Our team will review your information and contact you if anything else is needed before your appointment.

All payments for this appointment are final and non-refundable. Any day-of services or items (microchip, vaccines, e-collar/cone, labs, meds, special orders) are charged at time of service and are non-refundable once provided or dispensed. If the clinic reschedules, we’ll issue an account credit for the full amount.
