Doc Wayne is a sports therapy non- profit organization based in Boston. They work with children and teens of all genders who have suffered from trauma which has caused them to have mental health issues . Doc Wayne uses sport-based therapy to help treat mental health and break the stigma on getting help for mental illnesses.

For our soccer event we are teaming up with Positive Tracks. Their slogan is "Sweat for Good!" which connects to Doc Wayne because they are bringing people together through sports to discuss problems that we face in our world today. At this event be prepared to "Sweat for Good!"

By achieving our goal of $3,000,

Doc Wayne will be able to help children and many more manage their mental health by building onto their 68,000 youth in programs spread across in 25 countries. Every $300 we raise, will provide individual therapy for a child in need! Thank you for supporting a great cause!

I release and hold harmless the organizers, sponsors and volunteers involved in the planning and operation of the Doc Wayne x Positive Tracks soccer match and shoot outs from liability for any injury to me or my property and any and all claims in any manner arising from or associated with my participation, whether the liability, loss or damage is caused in whole or in part by their failure to use reasonable care in their activities associated with the Doc Wayne x Positive Tracks soccer match and shoot outs .

If I am under the age of 18 years, this release and hold harmless agreement shall be signed on my behalf by my parent or guardian.

In addition, I hereby give full permission for the photo/video/film use of my name and image in connection with this event.
