2. Fill out this Family Information Form. We estimate 5 minutes to fill out. This form asks you to choose the age range for your book bundle. If you'd like both please pay for the second bundle here!

Mark your calendar for the program kickoff on September 26th at 3:00-4:30pm, Melrose UU Church, 70 West Emerson Street, Melrose.
