Join us for Community Council

Cone Health’s Congregational & Community Nursing Program (CCNP) closes healthcare gaps and advances health equity by bringing care directly into congregations and communities. In this presentation, Laura Emery will share how the program provides one-on-one support, connects individuals to medical care and essential resources, and guides patients through the healthcare system.

The program promotes whole-person wellness—body, mind, and spirit—through health education, chronic disease prevention and management, screenings, vaccination clinics, and care coordination, helping reduce unnecessary ER visits and prevent repeat hospital stays.
