This year, we are proud to spotlight Better Health Partnership and celebrate the impact of the Better Health Pathways HUB in Cuyahoga County, a community-driven model that connects individuals and families to the services and support they need to live healthier, more stable lives.

The Carmella Rose Health Foundation was honored to be among the earliest members of the Better Health Pathways HUB, helping support a bold vision for care coordination that is now improving lives and strengthening communities across Cuyahoga County.
