Community Healthcare Resources (CHR) provides a revolutionary model of healthcare accessibility, providing clinicians across the United States with the tools to deliver preventive care and health education to underserved individuals and families directly in their own communities. Your support helps us ensure high-quality preventive health care is accessible to every community member.

Since its inception in 2019, CHR's team of clinicians have given more than $16 million in affordable and free health services to their communities. Now, with a growing network of over 100 participating clinicians nationwide, we are projected to deliver over $45 million in affordable and free health care to those with economic need in a single year by 2037.

Seven out of ten deaths are caused by chronic diseases, many of which can be prevented or managed with routine preventive care. From the Centers for Medicare & Medicaid Services:

"Preventive health care aims to block or delay the development of illness, disease, and other health-related issues and limit the negative impacts of chronic diseases. It includes activities that support early detection and management of potential problems before they develop into chronic conditions. Addressing health concerns early can improve long-term health outcomes.

Over half of all adult Americans live with at least one chronic disease. Conditions like heart disease, cancer, and diabetes are the leading causes of death and disability in the United States. They are often challenging and expensive to treat.

Identifying and addressing potential health risks early can often help people to have healthier, longer lives and avoid costly treatments. When the risk cannot be avoided, treating an issue earlier can slow down the severity of the disease and help people optimally manage their condition long-term.

Giving people the tools and knowledge to take control of their health not only leads to improved health outcomes, but also to long-term well being and financial savings."

CHR was founded by clinicians to better serve the public.
