We bring people, programs, and systems together so support is more coordinated, accessible, and effective. Through this work, individuals and families are not left to navigate alone—they are connected to the right support at the right time.

Throughout the event, you’ll hear from speakers who are part of this work every day—supporting individuals and families, strengthening collaboration across organizations, and investing in prevention efforts that create long-term stability. You’ll gain a deeper understanding of how coordinated care and community partnerships are improving access to services and shaping stronger outcomes across Southern Indiana.

This work matters because when systems operate in silos, people fall through the cracks. But when services are aligned and communities work together, support becomes more effective, barriers are reduced, and outcomes improve—not just for individuals and families, but for the entire community.
