NorthShore Health Centers began in 1995 with a simple but powerful vision: to ensure that every individual, regardless of income, background, or circumstance, could access quality healthcare close to home. From a single clinic in Portage, NorthShore has grown into a network of over 20 full-service and school-based health centers serving communities across Lake, Porter, LaPorte, and Jasper Counties.

As a Federally Qualified Health Center, NorthShore provides comprehensive care under one roof: medical, dental, women’s health, pediatrics, behavioral health, vision, pharmacy, and more. Each year, thousands of patients count on NorthShore not only for care, but for compassion, dignity, and hope.
