Basic Obedience Waiver

Castoff Pet Rescue (CPR) wants you to be satisfied with you participation in our canine obedience training course. For successful participation and enjoyment we ask that you understand and agree to the following requirements. By submitting this form you have read the policies, and agree to follow them.

1. The fee for this course is non-refundable.
2. The course runs 6 consecutive weeks and there are no make up classes.
3. Out of fairness to all participants, classes will begin promptly at the posted time.
4. Each owner is responsible for providing proper training equipment and suitable treats.
5. Please wear closed toed shoes.
6. Prepayment is required so we can focus on training not economics.
No prong collars or retractable leashes - Martingale, Buckle collars, or EZ walk will put you at the front of the class

I understand that there are inherent risks associated with the presence of dogs in training. I assume full responsibility for myself, my dog, any other handlers of my dog and any observers that attend class with me.

I agree that I have given full disclosure of any history of aggressive behavior by my dog and understand that failure to do so will result in being excused from class and no refund will be given.

I agree to hold harmless CPR and its staff, volunteers and Board of Directors for any harm that may happen to me or my pet during the course of training. I understand that trainers may intervene in dog-to-dog interactions and separate dogs as needed.

I understand that if my dog or I are a disruption to the class, the trainer reserves the right to excuse us. I grant full permission to CPR to use any photographs or video content of this event for any purposes.

NOTE: If your dog is reactive to people or other dogs, a group class may not be the best option. Please contact CPR at info@castoffpetrescue.org to discuss options for you. I have read and afree to follow the above policies by submitting this application.
