FREEDOM 5K RUN/WALK EVENT WAIVER: I and those others listed in My registration including minors (hereinafter collectively referred to as “I”, “Myself”, “Me”, “My” “the Registrants”) desire to participate in the Freedom 5k Virtual RunWalk (hereinafter referred to as “event”) on November 2, 2024, sponsored by Domestic Violence and Abuse Protection (hereinafter referred to as “DVAP”). By clicking “I HAVE READ, UNDERSTAND, AND AGREE TO THE EVENT WAIVER TERMS FOR MYSELF AND THE REGISTRANTS LISTED IN MY REGISTRATION”, I understand and acknowledge that DVAP will not allow Me/the Registrants to participate in the event without releasing and holding DVAP harmless from any liability arising out of participation in the event. I know that running a 5K is a potentially hazardous activity, and that I/the Registrants should not enter and run/walk unless I/the Registrants are medically and physically able. I have investigated the risks involved in the event and fully understand and assume such risks. Specifically, I understand and acknowledge that I/the Registrants may suffer or experience, among other things, personal injury or bodily damage, medical disabilities, loss or theft of personal property, abduction and even death. I request that DVAP allow Me/the Registrants to participate in the event, and in consideration thereof agree hereby to release and forever discharge DVAP, its officers and directors, and its employees, agents, and any persons volunteering on behalf of DVAP, from all actions, causes of action, injuries, claims, damages, costs or expenses of any kind, growing out of or related to the event in which I/the Registrants participate. I understand that this is a full and complete release of all injuries and damages which I/the Registrants may sustain as a result of My/the Registrants’ participation in any part of the event, regardless of the specific cause thereof. Minor Registrants: I confirm that I am the parent or legal guardian of any and all minors that are Registrants or, in the case of the only Registrant being a minor, that I will have My parent or legal guardian complete an Event Waiver on My behalf prior to participating in the event. This Agreement is binding on My/the Registrants’ heirs, successors and personal representatives. I grant permission to all of the foregoing to use My/the Registrants’ photographs, motion pictures, recordings or any other record of this event for any legitimate purposes. I understand that this event does not provide for refunds in the event of a cancellation, and by acknowledging this waiver, I consent that I/the Registrants are not entitled to a refund if the event is canceled before or during the event.
