At Chosen Hearts, we believe everyone deserves to feel seen, valued, and cared for. Every day, individuals and families in our community struggle to meet their most basic needs. Through our Community Outreach & Engagement Initiative, we are committed to offering comfort, dignity, and hope.

Our work goes beyond providing necessities — we offer compassion and connection. Every care package, warm meal, and shared conversation reminds our neighbors that they are not alone.

Last year with your help we were able to serve over 500 individuals and families in need — but there is still so much more to do. This year our goal is to raise $10,000 to expand our reach and serve even more people in need.

With this we can:

• Provide essential care packages with hygiene kits, socks, blankets, and personal care items to hundreds of individuals and families.

• Distribute nourishing meals and water, ensuring families and individuals have access to nutritious food and clean drinking water.

• Support vulnerable families by providing diapers, baby formula, strollers, and baby care essentials to parents and caregivers.

• Expand outreach services by hosting additional community events, conducting street outreach, and offering resource referrals.

• Empower connection by funding resources to provide on-the-spot assistance and link individuals to long-term support services.
