I’m reaching out to share an exciting new initiative—“Strong Foundations: Preventative Wellness for Resilient Lives”—designed to transform care delivery for military veterans, active-duty personnel, and low-income individuals through a culturally competent, trauma-informed, and fully integrated health model.

This 12-month renewable program blends primary care, behavioral health, health coaching, and case management to provide 26–52 meaningful touchpoints per year—guided by evidence-based strategies and centered around the social determinants of health--all based on trauma relief processes used in Special Operation Forces designed by Dr. Frueh.

We are currently in the planning and advisory phase and would be honored to gather your input, clinical insights, and recommendations. Your expertise could help shape key components of our staffing model, implementation logistics, and evaluation framework. Additionally, we are exploring grant partnerships and hospital support for operational funding, community outreach, and telehealth infrastructure.

Key program highlights include:

Quarterly preventive visits and monthly behavioral health screening
Veteran and military-informed mental health support
Social work and housing/job assistance
Culturally competent coaching and literacy around chronic illness
Peer support options and flexible telehealth access
