Triangle Care Management is a 501(c)(3) nonprofit providing LGBTQ+ affirming care management, advocacy, and support to aging LGBTQ+ individuals and people with disabilities. Our vision is simple: no senior LGBTQ+ community member should be left without care. We build personalized care pathways centered on comfort, safety, and dignity — without financial barriers standing in the way.

About Lesbians Embracing Aging (LEA)

Lesbians Embracing Aging (LEA) is the founding community within Triangle Care Management's Embracing Aging Program (EAP). LEA creates a network of socialization, information, and resources so lesbian women in the Coachella Valley can age well and stay connected — building personal care networks and circles of trust among members, with a path to care as they age. EAP welcomes other communities to build similar circles of support.
