Transitions Life Center is a nonprofit organization made up of parents, family members, educators, and community partners dedicated to improving the lives of adults with intellectual and developmental disabilities (IDD).

Founded in 2012, TLC has grown from a small summer program into a vibrant, inclusive community serving adults of all abilities. Most recently, TLC celebrated a ribbon -cutting marking the completion of a major campus expansion — a milestone that allows us to double the number of members we serve and expand the opportunities available to them.

This growth includes thoughtfully designed spaces such as a sensory room and a functional unit to support individuals with higher support needs, ensuring every member has access to meaningful, engaging programming in an environment built for them. TLC’s campus also features spaces that encourage movement, connection, and enrichment, including outdoor and multipurpose areas that support physical wellness, recreation, and social engagement.

At TLC, our members are adults who experience social, emotional, physical, and independence challenges. Each day, we focus on helping them strengthen independence, build friendships, explore their world, and grow in confidence — all within a supportive community that values dignity, inclusion, and belonging.
