Autism Support of Kent County is funded 100% by donations, and 100% of the money we raise stays in West Michigan.

LIABILITY WAIVER INFORMATION:

I hereby acknowledge and agree to participate voluntarily in the Walk for Autism organized by Autism Support of Kent County, which will take place on May 17 at the Special Olympics Unified Sports and Inclusion Center.

In consideration for being permitted to participate in this event, I, on behalf of myself, my heirs, executors, administrators, successors, and assigns, hereby release, waive, discharge, and covenant not to sue Autism Support of Kent County, its directors, officers, employees, volunteers, agents, and representatives (collectively referred to as "Released Parties") from any and all claims, liabilities, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, while participating in the event or while on the premises where the event is being conducted.

I understand and acknowledge that participating in the charity walk event involves certain risks, including but not limited to, the risk of bodily injury, harm, or death, and the risk of damage to or loss of property. I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me or any loss or damage to property owned by me as a result of participating in the event.

I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, liabilities, damages, losses, costs, or expenses, including reasonable attorney's fees, arising out of or related to my participation in the event or any breach of this agreement.

I acknowledge that I have carefully read this Waiver and Release of Liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I voluntarily agree to its terms without any inducement or assurance of any nature.

This Waiver and Release of Liability shall be binding upon me, my heirs, executors, administrators, successors, and assigns.

PHOTO RELEASE

I also understand that by entering the event venue, I may be in photographs or videos . I hereby consent to and authorize the use and reproduction by ASK of any and all photographs taken of me during the Walk for Autism event.

I understand that these photographs may be used for promotional purposes, including but not limited to, advertising, social media, website content, and other marketing materials, without compensation or further approval from me.
