At Hometown Connect we seek daily to fill the healthcare gaps in our community. This looks like taking in referrals from the community, learning of specific needs, working hard with our team through a thorough intake process and ultimately providing help to those who need it most.

We meet with people, pray with them, assess their needs through case management, connect individuals to helpful resources and facilities, track duplication of resources. We educate, help find housing, help find employment, help with budgeting, help find transportation, help provide food/warm meals, help with access to healthcare.

Our goal is to meet a physical
need and by doing so, introduce that person to the love and hope of Jesus.

Needs we see daily include:

Homelessness

Impact of addiction and drugs

Lack of nutritional foods

Lack of affordable healthcare

Medication co-pay assistance

Mental health accessibility

Greif counseling

Social work needs

Basic material needs.

Our goal is to help the WHOLE person in an effort to break cycles of hopelessness in our community. This takes an amazing amount of accountability and follow-up and working together to connect resources. We are committed to spending time on building real relationships that bring about lasting change. We are very thankful for any and all support from each of you!
